Companion Care Services
are provided by a caregiver who offers a
smiling face, a helping hand, ears to hear and a voice
to encourage your loved
one. Companion caregivers focus on providing the social
and physical stimulation to your loved ones that
encourages an active and engaging life style.

Personal Care
Services are provided by our Certified/Geriatric
Nursing Assistants and Home Health Aides who can provide
all the services under Companion Care but additionally,
provide those services that require a “hands on”
approach.

Care Management Services
are managed by the Director of Nursing and assist
families in trying to maneuver through the healthcare
maze. Care management services plan fully help families
ensure that their loved ones receive the services
required. The Care Manager completes a comprehensive
assessment of your loved one’s needs, provides
recommendations, options and coordinates referrals for
services. All of this is documented in the Care Plan
and that is reviewed and updated periodically. The Care
Manager monitors all services provided and is the
liaison between your loved ones, family members and
other service providers.

Extended Families
for Life
TM
is a service that is specifically designed for the
aging population and focuses on adults experiencing
depression and feelings of loneliness. It offers the
aging adult a social support system provided by
carefully selected families, called Extended Families,
living in near by communities and who have gone through
the same rigorous screening process as our caregivers.
The Extended Family is committed to being the “extended
family” of the aging adult, reinforces their Care Plan
and encourages a more active and vibrant life style.

Service begins with an assessment by our experienced
staff to suggest or rule out depression, followed by
recommendations for services.
-
Extended Families provide frequent visits and
ongoing phone contact with the aging adult and their
family members.
-
Gives long distance families peace of mind knowing
that their loved ones are being cared for, visited
regularly and supported by their Extended Family.
-
Rejuvenates the sense and feelings of “family” in
the aging adult who may have experienced significant
losses.
-
The aging adult visits with their Extended Family
during Thanksgiving, Christmas and other holidays
and family events.
-
The Extended Family participates in many social
activities with the aging adult that may include
church, outings, shopping, movies, plays, etc.
-
Extended Families also participate in
Multidisciplinary Team Meetings.
The
second component of Extended Families for Life allows
for support by local businesses, churches and
organizations that support people needing Tranquil
Gardens services, but do not have the capacity to pay
either through insurance or out of pocket. These
Corporate Extended Families sponsor people, generally of
lower income, so that they can receive the services
needed to remain comfortably at home.
All
of our Extended Families are amazing people and
corporations! They strong supporters of Tranquil
Gardens and have recognized the need for quality health
care services to people. Extended Families share
Tranquil Gardens philosophy that quality health care is
a basic human right, not a privilege and that everyone,
despite their financial status, deserves quality health
care.
